The conclusions must be seen to be based on sound evidence and must not be conjecture, although in some cases, where the cause of the problem has not been found, it is acceptable to comment on probable causes . The report should cover all stages of the investigation but the extent of the detail and depth will depend on the seriousness of the failure in question. A summary of the main findings and recommendations should also be prepared for use by directors/government officials etc. who do not want to go through all the technicalities of the problem and the investigation.
